The formula
The box corners are rotated in 3D and projected with a perspective factor; each visible face is filled by mapping your artwork across a subdivided grid for a perspective-correct look, then shaded by the lighting settings, with a soft floor shadow beneath. The result is drawn on an 800×800 canvas you can download.
